Exercise can help more people survive colon cancer

Colon cancer patients who are supported to do regular exercise are more likely to survive the disease, according to a trial we helped fund. 

As well as showing that exercise can cut the risk of death and cancer recurrence, the results highlight the importance of personalised support in helping people develop the right routine for them.

“For an intervention that isn’t a drug, exercise offers remarkable benefits for patients,” said Professor Charles Swanton, our chief clinician. “For some people with colon cancer, physical activity can be a game-changer that shifts the course of their recovery.”

Patients on the CO21 CHALLENGE trial had already finished their cancer treatment, which included surgery and chemotherapy, but they still faced a risk that their cancer could come back. The researchers wanted to see if increasing their activity levels would help them stay well.

Half of the people who took part received standard health education materials with guidance on exercise and healthy eating, while the rest were given regular support from a trainer to help them reach a weekly exercise target.

After five years, 80% of people supported to do structured exercise remained cancer free, compared with 74% of those who were given the standard booklets. At eight years, 90% of people with structured exercise programmes were still alive, up from 83% in the health education materials group.

Overall, the patients in the structured programme were 37% less likely to die in the eight years following their treatment.

Professor Vicky Coyle at the University of Belfast, who led the research in the UK, said: “During the trial, we supported people to reach their weekly exercise target in a way that worked for them. This could be a brisk walk for around 40 minutes every day, but some patients were also doing circuit classes, cycling, swimming, and many other activities.  

“Our study gives clear, encouraging evidence that physical activity can reduce the risk of cancer returning for some people with colon cancer. We now need to work with policymakers and healthcare providers to embed exercise into treatment plans where appropriate.” 

More than an academic exercise

Research has already shown that exercise can lower the risk of people developing certain cancers – including colon cancer, which is a form of bowel cancer.  

The CHALLENGE trial took a different approach, focusing on whether exercise could help people stay cancer free after finishing treatment for stage 2 or 3 colon cancer. All 889 participants were well enough to exercise when they joined, but none of them were meeting recommended activity levels. 

The structured exercise programme tested in the trial lasted three years and involved people regularly meeting with an expert physical activity consultant. For the first six months, participants received in-person coaching sessions every week. Over time, this was reduced to one session per month, either in person or online.

The goal was to increase exercise levels by at least the equivalent of a 30-minute jog every other day. On average, people in the structured exercise group did more than twice as much.

While the health materials offered valuable information about exercise, the results indicate that expert support makes a crucial difference. The trial’s physical activity consultants helped people find and develop exercise routines that matched their abilities, preferences and normal activity levels.

Margaret Tubridy, a 69-year-old from North Belfast who took part in the trial, said: “I had never exercised in my life, but with the right support and encouragement I was able to walk more and more. From there I started going to classes at the gym and now I do weight training twice a week, pole walking, and I’m in a walking group. I had never been to a gym before, but once I was shown how to use the machines and do the exercises, I loved it.  

“It’s been almost five years since my cancer diagnosis and if I meet anyone, I tell them I am doing great. Taking part in the trial helped me so much – I am stronger, fitter, and my anxiety is better. I never thought at 69 years of age I would be able to do all of this – I am chuffed to bits.”

Colon cancer, bowel cancer and early-onset cancer 

Colon cancer is the main form of bowel (colorectal) cancer – which is the fourth most common cancer in the UK. It starts in the colon: a part of the bowel that helps absorb water and turn food into poo. It’s the last part of the digestive system before the back passage, or rectum. 

Around 31,800 people are diagnosed with colon cancer in the UK every year. Although that number isn’t increasing overall, cases of bowel cancer are rising in adults under 50. These ‘early-onset’ bowel cancers are still relatively rare, but researchers have described the increase, which started in the 1990s, as a ‘global phenomenon’.  

While we’re working to understand how to prevent early-onset bowel cancers, the findings of the CHALLENGE trial suggest that exercise could become an important element of how doctors treat them. 

Exercise as cancer treatment

The CHALLENGE trial didn’t directly investigate the reasons for exercise’s positive effect on colon cancer survival, but researchers believe it could be linked to the way physical activity helps regulate hormone levels, reduce inflammation, and strengthen the immune system. 

Dr Joe Henson, an Associate Professor in Lifestyle Medicine at the University of Leicester who worked on the trial, said: “Leading the exercise sessions in Leicester, I saw firsthand that they reduced fatigue, lifted people’s mood and boosted their physical strength. We know that physical activity regulates several key biological processes that could explain these results, and further research will help us uncover why.” 

As that research continues, the CHALLENGE trial’s findings give policymakers and health services a clear reason to start focusing on using tailored exercise support to help more people survive cancer. 

Caroline Geraghty, our senior specialist information nurse, said: “Being active has many benefits, and this study shows that exercise doesn’t only improve wellbeing – it can also boost people’s chances of surviving colon cancer. 

“Everyone recovering from cancer should have access to tailored support to help them live longer, better lives. This trial has the potential to transform clinical practice, but only if health services have the necessary funding and staff to make it a reality for patients. 

“It’s important to remember that every cancer journey is different – starting new activities after treatment can feel overwhelming, and it may not be the right option for everyone. Take things at your own pace and speak to your doctor to discuss what is safe for you.” 

We supported the UK arm of the CHALLENGE trial through Stand Up to Cancer. The overall trial took place in six countries around the world, led by the Canadian Cancer Trials Group (CCTG) with the support of the Canadian Cancer Society.

    Many thanks to my nephew’s wife for sending me the link to this article. I think it makes a lot of sense – but is the level of resource needed for patient support likely to be readily available? I had surgery for bowel cancer in August 2023 and the chemotherapy follow up finished in January 2024. Since then my periodic reviews and screening results have been good – but although I wanted to – I have not been able to do much in the way of exercise due to fatigue, an injury and arthritic pain. I am also very overweight. It’s now 18 months since my chemotherapy finished and it really wiped me out so I was struggling and fatigued for a lot of 2024. I retired from work in December and after a nasty virus earlier this year I am at last starting to feel less tired although the arthritic pain can be a problem. It’s not ideal but I think/hope I can still benefit from an exercise and weight loss programme that starts now – 18 months on? Motivation is the hardest part and I need help. Our local GPs are overwhelmed and the hospital physiotherapy resource in my area is unlikely to be available to support me in this as they are so busy with people recovering from injuries. Being retired gives me more time and the local gym have kindly extended my membership – so I can make a start but it would be good to know if there are any resources in my area other than private gyms (which can be intimidating – full of young slim fit people) and slimming clubs (which from my experience can be really humiliating). Somewhere that I can get expert help and information – in order to devise a diet and exercise plan with realistic goals – tailored to my individual needs. I am sixty-seven – feeling at least ten years older! I don’t expect to get back to being 40 again – but I need to do something to improve my health. I’ve had a lucky escape from cancer so far and wish to keep it that way for as long as possible.

    I’ve never missed any screening appts.
    I was 68 when I had a smear test (approx July 2022) and results showed I had HPV and therefore would screened annually thereafter.
    After seeing GP 6 months later, as I had some symptoms, I was referred for hysteroscopy. The result was I had stage 4b vaginal cancer.
    In my opinion everyone who has had HPV positive should be investigated with urgency

    I was diagnosed with Stage 3 bowel cancer (colon) in January 2023,had surgery followed by adjuvant chemotherapy (CAPOX). Have tried unsuccessfully to get the article,the abstract just really tells us we have evidence but no.detail about what the exercise program followed was. Would really like to get the info re the structured exercise program followed in the trial and the health materials provided to the other group. Feel this would be extremely useful to all of us so we can try it ourselves since there’s really no formal exercise program post treatment just, get on with your life as we continue in the 5 year monitoring program(for which I’m truly grateful). I’m trying my best but would like to know I’m on the right track, if I wait for access, I’ll be almost 3 years post treatment as not till June 2026. Any help for the info gratefully received, seems a waste to have clear evidence we can act on if we don’t act now. Thanks.

    I was diagnosed with Bowel Cancer in Early 2021 with Surgery in August 2021. All the follow up Tests including Bloods and Colonoscopies have all been satisfactory. I played a lot of Squash from 1975 – 2000 as in 5 times per week. After 2000 I switched to Tennis 3 – 5 times per week. Also Spinning Classes twice per week. Yet with all this exercise I still developed Bowel Cancer. Regards, Roger Dean. 19/12/1943.

    I did the Step up to 30 in 2019 to fund raise for Bowel Cancer. I enjoyed it so much that I continued-walking a minimum of 30 minutes every day. Had a couple of breaks when I had to miss a day because of one of the terrible storms and another break when I had a heart attack🙄 . However, I went back to starting from Day 1 after my cardiac rehab and I m now on Day 1,238.
    My bowel cancer was in 2017(operation on Valentine s Day ❤️) and so far all clear😊

    I also found it quite confusing/annoying to try and calculate the amount of exercise required in the trial, so I made this online calculator to help me track. The study used “MET hours” as their metric, and the folks in the exercise group (that had improved overall survival) did on average 24 MET-hours per week. Hope this can be helpful: https://met-calculator-challenge.tiiny.site/

    It would have been helpful is this summary had said how the outcomes varied by age and sex. My husband is 82. He was fit and active before his surgery 8 weeks ago. What kind of exercise would suit him now?

Although this study didn’t look into how outcomes varied by age and sex, 49% of participants were men, the median age of participants was 61 and the oldest participant was 84. I’m not sure if your husband is in the same situation, but it’s also worth noting that participants also joined the trial shortly after finishing their post-surgery chemotherapy, which was the last stage of their treatment.

    Overall, international guidelines say it’s safe to be active during cancer treatment and after, and that people with cancer should try to be active and get back to their normal activities as soon as possible. Even so, it could be helpful for your husband to speak to his doctor about the best way for him to get back into exercising after his surgery, as he might need to take some precautions.     I would be very grateful if you could give more detailed information about how much exercise is needed for this beneficial effect, eg how many minutes of aerobic exercise per week, according to the results of the trial (eg in comparison to the usual advice of 150minutes of moderate physical exercise per week for general health)

    Hi Andrew,

    Thanks for your comment – we’re happy to explain more! The structured exercise programme tested in this trial was designed to increase people’s activity levels by about 10 metabolic equivalent task hours each week, which is roughly equivalent to or slightly above the standard recommendation of 150 minutes of moderate physical exercise. The researchers explained it as the equivalent of adding 45 to 60 minutes of brisk walking or 25 to 30 minutes of jogging three or four times per week.
